Model Release

I understand that the purpose of this project is to raise awareness of Breast Cancer, and the importance of encouraging women to have regular mammograms. I understand that a portion of any proceeds which may be acquired though this project, will be donated to a Breast Cancer Survivor Group selected by Claire.

I hereby give permission to Nancie J Photography to use my Breast Cancer Survivor photos and likeness in all forms and media including; a possible book, gallery showing, portfolio, trade, stock photography, editorial, altering without restrictions (other than what we have agreed to) and all other lawful purposes. I understand I am entitled to no compensation, and will not use these photos for any financial gain, or to release the images to anyone without permission from Nancie. I release the photographer (Nancie Jimenez) of all forms of claims and liability related to my photo usage. I am over 18 years of age, and I fully understand the terms of this release.  

In exchange for my participation I will be given copies of hi-resolution images and the print release to use my images for private purposes, which will delivered to me via Dropbox.
